A very bold skin made for tabulas... www.tabulas.com.

Preview doesn't quite work right, here is a better example.
In the code, I have added tags to help you navigate. // FOR THOSE UNFAMILIAR WITH HTML // if it says something like this in the code:

Then put your age where I put 15. Easy enough.

The code for the entry part is spaced below the normal code, so be careful when you cut & paste.
